Health Insurance
Optimal Chiro-Rehab Center is In-Network with Anthem Blue Cross Blue Shield (BCBS) PPO plans as well as Aetna plans.
Other insurance PPO plans such as Cigna and United Health Care have some type of Out-of-Network benefits. Since insurance plans and coverage varies widely, our staff will help verify and explain your benefits at no charge.
We suggest that before your initial visit to text or email us your insurance information so we may verify your benefits prior to your visit. Please include the following information:
1) Name of Insurance company
2) Patient name (as it is written on the insurance card)
3) Date of birth
4) ID/Membership number
5) Group number and/or enrollment code
6) Provider phone number (on the back of the card)
7) Policy Holder's name and date of birth (if different)
We also accept auto accident, workers compensation and personal injury cases.
No Insurance Options
If you do not have health insurance or are concerned that you may not be able to pay for your care in full, we may be able to help. Our team will work with you to create cost-efficient payment plans and offer care packages for multiple visit treatment plans.
